# Checklist Question & Answer Setup

The following options are available for [configuration for checklist](/en/lr/47738/) _Section Design_, _Question Design_, and _Available Answer Design_ records. Depending on your configuration, you may need to update [page layouts](/en/lr/26387/) to include available options:

## Section Design

* **Section Design Type**: Specifies whether the section is an _Introduction Section_ or _Question Section_.
* **Description**: Specifies a description of an _Introduction Section_ or _Question Section_.
* **Header**: Specifies the header text for an _Introduction Section_.
* **Weight %**: Specifies the weight percentage of a _Question Section_.
* **Randomize Questions**: Allows you to randomize the order of questions on the checklist displayed to users in a _Question Section_.

## Question Design {#question-design}

* **Question Design Type**: Specifies whether the question is a _Date_, _Multiple Choice_, _Number_, or _Text Question_.
* **Attachment Allowed (Attachment)**: Specifies whether users are allowed to attach files in a response.
* **Attachment Required (Attachment)**: Specifies whether users must attach files in a response. _Attachment Allowed (Attachment)_ must be set to **Yes** for this option to be set to **Yes**.
* **Comment Allowed**: Specifies whether users are allowed to add comments in a response.
* **Comment Required**: Specifies whether users must provide a comment in a response. _Comments Allowed_ must be set to **Yes** for this option to be set to **Yes**.
* **Order**: Specifies the order the questions will appear in each section. This option is also required for configuration for checklist _Available Answer Design_ records.
* **Question Text**: Provides the question to which users are asked to respond.
* **Required Response**: Specifies whether responding to the question is mandatory or optional for the user.
* **Maximum Selectable Answers** (_Multiple Choice Questions_ only): Specifies the maximum number of answers which users can select in a response. For _Checklist Answer Type_ records of _Radio_ and _Drop Down List_ type, this value is one (1). Ensure you create the appropriate number of corresponding _Available Answer Design_ records in order to change this value.
* **Checklist Answer Type** (_Multiple Choice Questions_ only): Specifies whether the answer type for the multiple choice question is _Radio_, _Drop Down List_, or _Checkbox_.
* **Documents Allowed**: Specifies whether users can add related version-specific documents in their response at the Question level for _Number_, _Text_, _Date_, and _Multiple Choice Questions_ or at the _Answer_ level for _Multiple Choice Questions_. For multiple choice questions, this option is also available for configuration for checklist _Available Answer Design_ records. A _Multiple Choice Question_ cannot have _Documents Allowed_ set to _Yes_ at both the _Question_ and _Answer_ levels. Users can add a maximum of ten (10) documents.
* **Documents Required**: When _Documents Allowed_ is set to **Yes**, this option specifies whether users must select a _Single Document_, _Multiple Documents_, or whether selecting documents is _Not Required_ (optional) as part of their response. For multiple choice questions, this option is also available for configuration for checklist _Available Answer Design_ records. For multiple choice questions, ensure you configure this option only for the _Question Design_ record or the _Available Answer Design_ record but not for both. Users can add a maximum of ten (10) documents.
* **Reference Media**: Allows you to add an image to the question for the checklist respondent to view. You can also use this field on the _Available Answer Design_ record.
* **Comment Type**: Specifies whether users can enter comments in [Text](/en/lr/15057/#text-field), [Long Text](/en/lr/15057/#long-text), or [Rich Text](/en/lr/15057/#rich-text-field) format.
* **Randomize Answers**: Allows you to randomize the order of answers on the checklist displayed to users.
* **Question Help Text**: You can optionally provide up to 300 characters of text per question to guide checklist respondents in [how to answer a question](/en/lr/49033/#respond). When configured for a question, users can hover over the question to see the question mark icon (**?**) in the top right corner of the question. Users can then click the question mark icon (**?**) to display the guidance text. You can configure this option for _Question Design_ records (including [_Library Question_](/en/lr/52824/#question_lib) records). To use this option, you may need to update the [page layout](/en/lr/26387/) for _Question Design_ records. If your configuration includes custom object types for _Question Design_ records, you need to [assign](/en/lr/32857/#assign) the _Question Text_ field (`question_help_text__sys`) to the appropriate custom object types.
* **Reference Document(s)**: You can optionally add up to five (5) documents per question with more related information. When configured, a reference document appears as a link that the respondent can click to view the document in a pop-up window.
* **Minimum Answer Value** (_Number Question_ only): Specifies the lower range limit for valid responses as a number between -100,000,000 to 100,000,000, including up to nine (9) decimal places.
* **Maximum Answer Value** (_Number Question_ only): Specifies the upper range limit for valid responses as a number between -100,000,000 to 100,000,000, including up to nine (9) decimal places. The _Maximum Answer Value_ must be greater than the _Minimum Answer Value_ for number questions.
* **Decimal Places** (_Number Question_ only): Specifies the number of decimal places allowed in response to a number question as an integer from zero (0) to nine (9). The default if you do not specify a value for this field is zero (0) and checklist respondents must enter an integer (no decimal places) in number question responses.

* **Action Items Allowed** (QualityOne only): Specifies whether users can create action items from their response at the Question level for Number, Text, Date, and Multiple Choice questions at the Question level or at the Answer level for Multiple Choice questions. For Multiple Choice questions, this option is also available for configuration for checklist _Available Answer Design_ records. Note that a _Multiple Choice Question_ cannot have _Action Items Allowed_ set to _Yes_ at both the Question and Answer levels.
* **Action Items Required** (QualityOne only): When _Action Items Allowed_ is set to **Yes**, this option specifies whether users must create action items as part of their response to a checklist. For Multiple Choice questions, this option is also available for configuration for checklist _Available Answer Design_ records. Note that a _Multiple Choice Question_ cannot have _Action Items Required_ set to _Yes_ at both the Question and Answer levels.
* **Suggested Actions** (QualityOne only): When _Action Items Allowed_ is set to **Yes** at either the Question or Answer level, you can optionally use this field to provide guidance to users completing the checklist regarding the action items they should create. When configured, a question mark appears beside the _Action Items_ label in the Checklist Respondent UI which users can click to view the suggested actions. For Multiple Choice questions, this option is also available for configuration for checklist _Available Answer Design_ records. When configured at the Question level, the suggested action question mark and hovercard appears regardless of the respondent's answer. When configured at the Answer level, the suggested action question mark and hovercard appears only when the user selects the answer for which **Suggested Actions** is configured. For Multiple Choice questions of type _Checkbox_ with _Maximum Selectable Answers_ specified as a number greater than one (1), a single hovercard will display the combined suggested action text from each answer option that the user selects. 

## Available Answer Design

* **Answer Text**: Specifies the question's answer.
* **Score** (_Multiple Choice Questions_ only): Score is calculated against the _Weight%_ option that is set up for each section in the _Question_ level and by using the points value of each answered question in the _Answer_ level.

* **Approver Type** (QualityOne only): Specifies the type of user role needed to approve the checklist. Multiple user roles can also be picked. If a user selects an answer that has a value for approver type, that value will be copied to the checklist record when completing the checklist. This option is available for configuration only at the Answer level. 

## Checklist Limits

The following maximums apply to checklists:

  * Sections per checklist: 20
  * Questions per section: 50
  * Answers per question: 20
